We have an exciting opportunity for someone to join the Somerset Independence Plus service, a partnership between Local Authorities across Somerset and Somerset County Council.

The Hospital Resettlement Coordinator is an additional post for 18 months supporting people in a hospital setting based at Musgrove Park Hospital and may require travel to Community Hospitals.

The post supports patients whose discharge is being or is likely to be severely compromised by difficulties relating to their housing needs. The post will assist with reducing the overall average length of stay and total number of occupied bed days on in-patient wards, by providing a comprehensive service to patients who have been identified with the need for housing related support.

Working to link health and social care, housing and other appropriate services this post will produce appropriate discharge plans for customers. You will work alongside HIS and ward colleagues to achieve a smooth transition from hospital discharge to the home setting through the resettlement plan, with the aim of preventing re-admissions into hospital.

Finding suitable solutions is the key to this role. If you feel you have the skills to problem solve, then this could be the job for you.

The successful candidate will have previous experience of working with housing legislation, priority need and homelessness. Candidates should also ideally have experience of working in a clinical setting and with mental health issues in a work environment. Excellent communication and negotiation skills are also required.

There is a need to build on the Partnership's continued customer satisfaction, improving performance, achieving efficiencies and enhancing service resilience. As the role will be working in the hospitals and working with vulnerable clients and partner organisations professionalism and excellent customer care are essential attributes for this post.
